A checklist for registering a player for the season.

The player must have a Sports Physical (signed by a MD or DO within the last 2 years) on file with Corvallis Girls Lacrosse. If the player has a current Sports Physical on file with her school, please ask the Athletic Department for a copy.

List of equipment each player must have. CGL has a limited number of sticks and goggles that can be checked out for the season for those who are new or do not wish to buy their own. No jewelry is allowed.

Players wishing to be considered for a scholarship to help offset the dues/registration fee should complete this application form. Bring the completed form to a pre-season/registration meeting or email it to corvallisgirlslax@gmail.com.

As a volunteer-run organization, we need your help to make the season a success. Each player's family is asked to sign up for at least one volunteer activity.
